Complete this form in order to ensure that your child is photographed on picture day. Please complete a separate form for each child.

Don’t worry about drapes or tuxedos; we provide them. However, boys should bring a solid white t-shirt to wear underneath, and girls should bring a spaghetti strap top. (Tip: A pearl necklace makes a great accessory for the little girls.) Feel free to wear any accessory of your choice.
